This protein forms membrane pores that allow the release of granzymes and subsequent cytolysis of target cells. Whether pore formation occurs in the plasma membrane of target cells or in an endosomal membrane inside target cells is subject to debate. Mutations in this gene are associated with a variety of human disease including diabetes, multiple sclerosis, lymphomas, autoimmune lymphoproliferative syndrome (ALPS), aplastic anemia, and familial hemophagocytic lymphohistiocytosis type 2 (FHL2), a rare and lethal autosomal recessive disorder of early childhood. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251458 control chromosomes (gnomAD). c.916G>T has been reported in the literature in multiple individuals affected with Familial Hemophagocytic Lymphohistiocytosis (e.g. Grossman_2005, Trizzino_2008, Gadoury-Levesque_2020, Shabrish_2021). These data indicate that the variant is very likely to be associated with disease. These publications also reported experimental evidence characterizing patient derived cells, and demonstrated barely detectable perforin expression and and minimal NK-cell killing activity (e.g. Grossman_2005, Shabrish_2021). The following publications have been ascertained in the context of this evaluation (PMID: 15840696, 17873118, 32542393, 33746956). ClinVar contains an entry for this variant (Variation ID: 2678060).
