The p.P2699R variant (also known as c.8096C>G), located in coding exon 54 of the ATM gene, results from a C to G substitution at nucleotide position 8096. The proline at codon 2699 is replaced by arginine, an amino acid with dissimilar properties. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251272 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.8096C>G in individuals affected with Breast Cancer and no experimental evidence demonstrating its impact on protein function have been reported. This variant is denoted ATM c.8096C>G at the cDNA level, p.Pro2699Arg (P2699R) at the protein level, and results in the change of a Proline to an Arginine (CCA>CGA). This variant has not, to our knowledge, been published in the literature as either a germline pathogenic or benign variant. However, it has been reported as a somatic variant in a mantle cell lymphoma (Greiner 2006). ATM Pro2699Arg was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, suggesting it is not a common benign variant in these populations. Since Proline and Arginine differ in polarity, charge, size or other properties, this is considered a non-conservative amino acid substitution. ATM Pro2699Arg occurs at a position that is conserved across species and is not located in a known functional domain. In silico analyses predict that this variant is probably damaging to protein structure and function. Based on currently available evidence, it is unclear whether ATM Pro2699Arg is a pathogenic or benign variant. We consider it to be a variant of uncertain significance. -
