This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. Variant summary: ATM c.8476_8477dupAA (p.Asn2826LysfsX32) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant was absent in 251310 control chromosomes. c.8476_8477dupAA has been reported in the literature in individuals affected with Ataxia-Telangiectasia (Stankovic_1998). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Two clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic.

The c.8476_8477dupAA pathogenic mutation, located in coding exon 57 of the ATM gene, results from a duplication of AA at nucleotide position 8476, causing a translational frameshift with a predicted alternate stop codon (p.N2826Kfs*32). This alteration was reported in a cohort of individuals diagnosed with ataxia-telangiectasia (A-T) (Stankovic T et al. Am. J. Hum. Genet., 1998 Feb;62:334-45). Of note, this alteration is also known as 8478 ins AA in published literature. In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.
