This sequence change replaces arginine, which is basic and polar, with leucine, which is neutral and non-polar, at codon 264 of the IGHMBP2 protein (p.Arg264Leu). This variant is present in population databases (rs777575504, gnomAD 0.006%). This missense change has been observed in individual(s) with Charcot-Marie-Tooth disease (PMID: 26392352; Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 466598). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt IGHMBP2 protein function with a positive predictive value of 95%. Variant summary: IGHMBP2 c.791G>T (p.Arg264Leu) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant allele was found at a frequency of 8e-06 in 251468 control chromosomes (gnomAD). c.791G>T has been observed in individuals affected with Charcot-Marie-Tooth disease axonal type 2 (Antoniadi_2015, Labcorp (formerly Invitae)). These data indicate that the variant is likely to be associated with disease. A different variant affecting the same codon has been classified as likely pathogenic by our lab (c.791G>A, p.Arg264His), supporting the critical relevance of codon 264 to IGHMBP2 protein function. The following publication has been ascertained in the context of this evaluation (PMID: 26392352). ClinVar contains an entry for this variant (Variation ID: 466598). Based on the evidence outlined above, the variant was classified as pathogenic.
