Variant summary
Our verdict is Pathogenic. The variant received 18 ACMG points: 18P and 0B. PM1PM2PM5PP3_StrongPP5_Very_Strong
The NM_014365.3(HSPB8):c.421A>G(p.Lys141Glu) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. K141T) has been classified as Likely pathogenic.

Published functional studies demonstrate significantly decreased neuronal development and neurite formation (PMID: 15122253, 20538880, 20858900); Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant does not alter protein structure/function; This variant is associated with the following publications: (PMID: 21985219, 15122253, 20858900, 20538880, 26718575, 20157854, 28251916, 31827005) -

Charcot-Marie-Tooth disease axonal type 2L Pathogenic:1
This sequence change replaces lysine, which is basic and polar, with glutamic acid, which is acidic and polar, at codon 141 of the HSPB8 protein (p.Lys141Glu). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with distal hereditary motor neuropathy (dHMN) (PMID: 15122253, 26718575, 28251916). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 2618). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects HSPB8 function (PMID: 15122253, 20538880, 21985219). For these reasons, this variant has been classified as Pathogenic. -
