This missense variant replaces histidine with leucine at codon 2623 of the BRCA2 protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with hereditary cancer in the literature. This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.H2623L variant (also known as c.7868A>T), located in coding exon 16 of the BRCA2 gene, results from an A to T substitution at nucleotide position 7868. The histidine at codon 2623 is replaced by leucine, an amino acid with similar properties. Other variant(s) at the same codon, p.H2623R (c.7868A>G), p.H2623Y (c.7867C>T), have been shown to be non-functional in homology-directed DNA repair (HDR) assays, structurally deleterious, and at least one segregated with disease in multiple families (Guidugli L et al. Am. J. Hum. Genet. 2018 02;102:233-248; Hart SN et al. Genet. Med. 2019 01;21:71-80; Yang H et al. Science. 2002 Sep;297:1837-48; Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -

Therefore, this variant has been classified as Likely Pathogenic. This variant disrupts the His2623 amino acid residue in BRCA2. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 26023681, 29394989, 31409081, 32444794, 33609447; Invitae; external communication). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt BRCA2 protein function. ClinVar contains an entry for this variant (Variation ID: 922959). This variant has not been reported in the literature in individuals affected with BRCA2-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces histidine, which is basic and polar, with leucine, which is neutral and non-polar, at codon 2623 of the BRCA2 protein (p.His2623Leu). -
