The p.A3122P variant (also known as c.9364G>C), located in coding exon 24 of the BRCA2 gene, results from a G to C substitution at nucleotide position 9364. The alanine at codon 3122 is replaced by proline, an amino acid with highly similar properties. This alteration has been identified in multiple individuals diagnosed with ovarian cancer (Barbosa A et al. Cancers (Basel), 2020 Sep;12:; Peixoto A et al. Front Oncol, 2020 Jul;10:1318). This variant was non-functional in a homology-directed DNA repair (HDR) assay (Richardson et al. Am J Hum Genet 2021 03;108(3):458-468). This amino acid position is highly conserved in available vertebrate species. This alteration is also predicted to destabilize the local structure and disrupt the protein binding ability of BRCA2 (Yang H et al. Science 2002 Sep;297:1837-48; Marston NJ et al. Mol. Cell. Biol. 1999 Jul;19:4633-42; Ambry internal data). In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -

This sequence change replaces alanine, which is neutral and non-polar, with proline, which is neutral and non-polar, at codon 3122 of the BRCA2 protein (p.Ala3122Pro). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with BRCA2-related cancers (PMID: 32850417; Invitae). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 439019). Advanced modeling performed at Invitae incorporating data from internal and/or published experimental studies (PMID: 33609447) indicates that this missense variant is expected to disrupt BRCA2 function with a positive predictive value of 95%. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -

It has not been reported in large, multi-ethnic general populations (http://gnomad.broadinstitute.org). In the published literature, the variant has been reported in individuals with ovarian cancer (OC) (PMIDs: 33008098 (2020), 32850417 (2020)). One functional analysis reported that this variant lacks homology-directed repair (HDR) activity, but additional functional studies are needed to conclude the effect of this variant on gene or gene product. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded conflicting predictions that this variant is benign or damaging. Based on the available information, we are unable to determine the clinical significance of this variant. -
