ATP7B (HGNC:870): (ATPase copper transporting beta) This gene is a member of the P-type cation transport ATPase family and encodes a protein with several membrane-spanning domains, an ATPase consensus sequence, a hinge domain, a phosphorylation site, and at least 2 putative copper-binding sites. This protein is a monomer, and functions as a copper-transporting ATPase which exports copper out of the cells, such as the efflux of hepatic copper into the bile. Alternate transcriptional splice variants, encoding different isoforms with distinct cellular localizations, have been characterized. Mutations in this gene have been associated with Wilson disease which is characterized by copper accumulation. [provided by RefSeq, Dec 2019]

The missense c.3971A>G p.Asn1324Ser variant in the ATP7B gene which is in the mutational hotspot has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. Another missense variant at this position has been reported previously in association with Wilson disease Zarina, Agnese et al.,2017. This variant is reported with the allele frequency 0.0004% in the gnomAD Exomes . It is submitted to ClinVar as Likely Pathogenic. The amino acid Asparagine at position 1324 is changed to a Serine changing protein sequence and it might alter its composition and physico-chemical properties. Multiple lines of computational evidence Polyphen - Damaging, SIFT – Damaging and MutationTaster - Disease causing predict a damaging effect on protein structure and function for this variant.The amino acid Asparagine in ATP7B is predicted as conserved by GERP++ and PhyloP across 100 vertebrates. For these reasons, this variant has been classified as Likely Pathogenic.

This sequence change replaces asparagine, which is neutral and polar, with serine, which is neutral and polar, at codon 1324 of the ATP7B protein (p.Asn1324Ser). This variant is present in population databases (rs760285767, gnomAD 0.0009%). This missense change has been observed in individuals with Wilson disease (PMID: 22677543, 32043565, 35220961). ClinVar contains an entry for this variant (Variation ID: 1236173). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt ATP7B protein function with a positive predictive value of 80%. This variant disrupts the p.Asn1324 amino acid residue in ATP7B. Other variant(s) that disrupt this residue have been observed in individuals with ATP7B-related conditions (PMID: 28717664), which suggests that this may be a clinically significant amino acid residue. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.

To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with Wilson disease in the literature. This variant has been identified in 1/249578 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
