The variant is not observed in the gnomAD v2.1.1 dataset. It is located in a mutational hot spot and/or well-established functional domain in which established pathogenic variants have been reported. Missense changes are a common disease-causing mechanism. In silico tool predictions suggest damaging effect of the variant on gene or gene product (REVEL: 0.83; 3Cnet: 0.87). Same nucleotide change resulting in same amino acid change has been previously reported as pathogenic/likely pathogenic with strong evidence (ClinVar ID: VCV000003001). A different missense change at the same codon (p.Arg163Trp) has been reported as pathogenic/likely pathogenic with strong evidence (ClinVar ID: VCV000002997). Therefore, this variant is classified as Pathogenic according to the recommendation of ACMG/AMP guideline. -

Located within the helix initiation motif in the 1A domain, a region intolerant to change (Chamcheu et al., 2011); In silico analysis, which includes protein predictors and evolutionary conservation, supports a deleterious effect; In silico analysis, which includes splice predictors, suggests this variant may impact gene splicing. In the absence of RNA/functional studies, the actual effect of this sequence change is unknown.; Not observed at significant frequency in large population cohorts (gnomAD); This variant is associated with the following publications: (PMID: 22678789, 22584502, 7512862, 24862219, 22402445, 31074163, 22262370, 33914963, 19874353, 30666268) -

This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 163 of the KRT9 protein (p.Arg163Gln). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with epidermolytic palmoplantar keratoderma (PMID: 30666268, 33914963). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 3001). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt KRT9 protein function with a positive predictive value of 80%. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. This variant disrupts the p.Arg163 amino acid residue in KRT9. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 22262370). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic. -
