This pathogenic variant is denoted BRCA1 c.1326T>A at the cDNA level and p.Cys442Ter (C442X) at the protein level. The substitution creates a nonsense variant, which changes a Cysteine to a premature stop codon (TGT>TGA), and is predicted to cause loss of normal protein function through either protein truncation or nonsense-mediated mRNA decay. This variant, also known as BRCA1 1445T>A using alternate nomenclature, has been reported in an individual with a family history of breast and/or ovarian cancer and an individual with multiple primary tumors diagnosed under the age of 60 (Kadouri 2004, Whitworth 2014). This variant is considered pathogenic.
Hereditary cancer-predisposing syndrome Pathogenic:2
This variant changes 1 nucleotide in exon 10 of the BRCA1 gene, creating a premature translation stop signal. This variant is expected to result in an absent or non-functional protein product. To our knowledge, functional studies have not been reported for this variant. This variant has been detected in at least three individuals affected with breast or ovarian cancer (PMID: 28145423, 29337092; Color internal data), in an individual affected with multiple primary cancer (PMID: 25248401) and in at least two suspected hereditary breast and ovarian cancer families (PMID: 14648706, 29446198). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Loss of BRCA1 function is a known mechanism of disease (clinicalgenome.org). Based on the available evidence, this variant is classified as Pathogenic.
The p.C442* pathogenic mutation (also known as c.1326T>A), located in coding exon 9 of the BRCA1 gene, results from a T to A substitution at nucleotide position 1326. This changes the amino acid from a cysteine to a stop codon within coding exon 9. This mutation (designated as 1445T>A) was identified in one non-Ashkenazi individual from the UK with a family history of breast or ovarian cancer (Kadouri L et al. Int. J. Cancer 2004 Jan;108:399-403). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.
Hereditary breast ovarian cancer syndrome Pathogenic:2
Variant summary: BRCA1 c.1326T>A (p.Cys442X) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. The variant was absent in 250930 control chromosomes. c.1326T>A has been reported in the literature in individuals affected with Hereditary Breast And Ovarian Cancer Syndrome (Kadouri_2004, Mattocks_2010, Whitworth_2014). These data indicate that the variant is likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 20167696, 25248401, 14648706). Six submitters have cited clinical-significance assessments for this variant to ClinVar after 2014. All submitters classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic.

Loss-of-function variants in BRCA1 are known to be pathogenic (PMID: 20104584). This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with breast cancer (PMID: 14648706, 29446198). This variant is also known as c.1445T>A. ClinVar contains an entry for this variant (Variation ID: 54202). For these reasons, this variant has been classified as Pathogenic.

The BRCA1 c.1326T>A variant is predicted to result in premature protein termination (p.Cys442*). This variant was reported as a causative variant in an individual with multiple primary malignant tumors (Supplementary Table 3, Whitworth et al. 2014. PubMed ID: 25248401) and in multiple families suspected to have hereditary breast and ovarian cancer (Supplementary Table 1, Rebbeck et al. 2018. PubMed ID: 29446198). This variant has not been reported in a large population database, indicating this variant is rare. This variant is interpreted as pathogenic in ClinVar (https://www.ncbi.nlm.nih.gov/clinvar/variation/54202/). Nonsense variants in BRCA1 are expected to be pathogenic. This variant is interpreted as pathogenic.
