This sequence change replaces glutamic acid, which is acidic and polar, with glutamine, which is neutral and polar, at codon 400 of the AXIN2 protein (p.Glu400Gln). This variant is present in population databases (no rsID available, gnomAD 0.004%). This variant has not been reported in the literature in individuals affected with AXIN2-related conditions. ClinVar contains an entry for this variant (Variation ID: 464523). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

The AXIN2 c.1198G>C (p.Glu400Gln) variant has been reported in the published literature in the somatic state in a prostate cancer biopsy (PMID: 31874108 (2020)). The frequency of this variant in the general population, 0.000035 (4/112938 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. The p.E400Q variant (also known as c.1198G>C), located in coding exon 4 of the AXIN2 gene, results from a G to C substitution at nucleotide position 1198. The glutamic acid at codon 400 is replaced by glutamine, an amino acid with highly similar properties. This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
