The MLH1 c.979C>G (p.Gln327Glu) variant has been reported in the published literature in individuals affected with breast cancer (PMID: 32091409 (2020)) and acute lymphoblastic leukemia (PMID: 26580448 (2015)). The frequency of this variant in the general population, 0.000013 (2/152206 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is damaging. Based on the available information, we are unable to determine the clinical significance of this variant. -
Hereditary cancer-predisposing syndrome    Uncertain:2 
The p.Q327E variant (also known as c.979C>G), located in coding exon 11 of the MLH1 gene, results from a C to G substitution at nucleotide position 979. The glutamine at codon 327 is replaced by glutamic acid, an amino acid with highly similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this alteration remains unclear. -

Variant summary: MLH1 c.979C>G (p.Gln327Glu) results in a conservative amino acid change located in the DNA mismatch repair protein, S5 domain 2-like (IPR013507) of the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251388 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.979C>G has been reported in the literature in one individual affected with B-Lineage ALL E2A-PBX1 Subtype (Zhang_2015). The report does not provide unequivocal conclusions about association of the variant with Lynch Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication have been ascertained in the context of this evaluation (PMID: 26580448). ClinVar contains an entry for this variant (Variation ID: 141891).
