MLH1 (HGNC:7127): (mutL homolog 1) The protein encoded by this gene can heterodimerize with mismatch repair endonuclease PMS2 to form MutL alpha, part of the DNA mismatch repair system. When MutL alpha is bound by MutS beta and some accessory proteins, the PMS2 subunit of MutL alpha introduces a single-strand break near DNA mismatches, providing an entry point for exonuclease degradation. The encoded protein is also involved in DNA damage signaling and can heterodimerize with DNA mismatch repair protein MLH3 to form MutL gamma, which is involved in meiosis. This gene was identified as a locus frequently mutated in hereditary nonpolyposis colon cancer (HNPCC). [provided by RefSeq, Aug 2017]

The p.E679* variant (also known as c.2035G>T), located in coding exon 18 of the MLH1 gene, results from a G to T substitution at nucleotide position 2035. This changes the amino acid from a glutamic acid to a stop codon within coding exon 18. This mutation was seen along with MLH1 copy-neutral loss of heterozygosity (CN-LOH) in an MSI-H colorectal tumor demonstrating loss of MLH1 and PMS2 by IHC (Gray PN et al. Oncotarget, 2018 Apr;9:20304-20322). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -
