Variant summary

Our verdict is Likely benign. The variant received -3 ACMG points: 4P and 7B. PM1PM5BP4_ModerateBP6BS1

The NM_004168.4(SDHA):c.1661G>A(p.Arg554Gln) variant causes a missense, splice region change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.0000558 in 1,611,472 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 3/3 splice prediction tools predict no significant impact on normal splicing. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R554L) has been classified as Uncertain significance.

Genes affected
SDHA (HGNC:10680): (succinate dehydrogenase complex flavoprotein subunit A) This gene encodes a major catalytic subunit of succinate-ubiquinone oxidoreductase, a complex of the mitochondrial respiratory chain. The complex is composed of four nuclear-encoded subunits and is localized in the mitochondrial inner membrane. Mutations in this gene have been associated with a form of mitochondrial respiratory chain deficiency known as Leigh Syndrome. A pseudogene has been identified on chromosome 3q29. Alternatively spliced transcript variants encoding different isoforms have been found for this gene. [provided by RefSeq, Jun 2014]

ACMG classification

Classification was made for transcript

Our verdict: Likely_benign. The variant received -3 ACMG points.

PM1
In a hotspot region, there are 2 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 1 benign, 33 uncertain in NM_004168.4
PM5
Other missense variant is known to change same aminoacid residue: Variant chr5-251100-C-T is described in ClinVar as Likely_pathogenic. ClinVar VariationId is 8742.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.
BP4
Computational evidence support a benign effect (MetaRNN=0.070874125).
BP6
Variant 5-251101-G-A is Benign according to our data. Variant chr5-251101-G-A is described in ClinVar as Conflicting_classifications_of_pathogenicity. ClinVar VariationId is 239655.
BS1
Variant frequency is greater than expected in population eas. GnomAd4 allele frequency = 0.0000854 (13/152314) while in subpopulation EAS AF = 0.00193 (10/5182). AF 95% confidence interval is 0.00105. There are 0 homozygotes in GnomAd4. There are 8 alleles in the male GnomAd4 subpopulation. Median coverage is 33. This position passed quality control check.
