Variant summary

Our verdict is . The variant received 6 classification points (ACMG Germline Pathogenicity v2019): 6P and 0B. PM1PM2PM5

The NM_000500.9(CYP21A2):c.844G>A (p.Val282Met) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a cumulative frequency of 0.00000659 (AC=1) in the gnomAD database across 151,774 control chromosomes (no homozygotes observed). The grpmax filtering allele frequency (95% CI) is 0.0000659. Note: a gnomAD entry for this variant shows a statistical allele-bias signature, consistent with mosaic/somatic contamination (e.g. age-related clonal hematopoiesis) rather than true inherited population frequency — this frequency should not be read as evidence of a common, benign germline variant. Splicing prediction tools (SpliceAI) predict no significant impact on normal splicing. No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar. A different missense at the same amino acid position has been reported as Pathogenic/Likely Pathogenic in ClinVar: p.V282L: Pathogenic/Likely_pathogenic (ClinVar VariationId 12151, 2 stars) Other variants at the same amino acid position have been reported in ClinVar (not pathogenic): p.V282L: not_provided (ClinVar VariationId 65610)

Genes affected
CYP21A2 (HGNC:2600): (cytochrome P450 family 21 subfamily A member 2) This gene encodes a member of the cytochrome P450 superfamily of enzymes. The cytochrome P450 proteins are monooxygenases which catalyze many reactions involved in drug metabolism and synthesis of cholesterol, steroids and other lipids. This protein localizes to the endoplasmic reticulum and hydroxylates steroids at the 21 position. Its activity is required for the synthesis of steroid hormones including cortisol and aldosterone. Mutations in this gene cause congenital adrenal hyperplasia. A related pseudogene is located near this gene; gene conversion events involving the functional gene and the pseudogene are thought to account for many cases of steroid 21-hydroxylase deficiency. Two transcript variants encoding different isoforms have been found for this gene. [provided by RefSeq, Jul 2008]

Classification according to ACMG Germline Pathogenicity v2019

Classification was made for transcript

Our verdict: Likely_pathogenic. The variant received 6 points.

PM1
Missense neighbourhood hotspot (≥2 P/LP within ±8 AA, OR ≥ 5 vs. background) — PM1; In-domain: 0 pathogenic, 0 benign rare missense variants.; ±8 AA neighbourhood: 2 pathogenic, 0 benign (OR vs. background: 25.0).
PM2
Very rare in gnomAD for AR/unknown gene (popmax AF < threshold/10) — PM2; GnomAD popmax AF = 0.0000659 — very rare for AR gene (base 0.001, raised to 0.005 by highest known P/LP ClinVar AF 0.00716) — PM2 moderate.
PM5
Different pathogenic missense at same AA residue in ClinVar (PM5); ClinVar contains a germline Pathogenic/Likely Pathogenic entry at the same amino acid position with a different amino acid change: p.V282L: Pathogenic/Likely_pathogenic (ClinVar VariationId 12151, 2 stars); Other variants at the same amino acid residue (not pathogenic): p.V282L: not_provided (ClinVar VariationId 65610)

⚠️ The allele balance in gnomAD4 Exomes is highly skewed from 0.5 (p-value = 0.00000000323577), which strongly suggests a high chance of mosaicism in these individuals.
