POR (HGNC:9208): (cytochrome p450 oxidoreductase) This gene encodes an endoplasmic reticulum membrane oxidoreductase that is essential for multiple metabolic processes, including reactions catalyzed by cytochrome P450 proteins for metabolism of steroid hormones, drugs and xenobiotics. The encoded protein has a flavin adenine dinucleotide (FAD)-binding domain and a flavodoxin-like domain which bind two cofactors, FAD and FMN, that allow it to donate electrons directly from NADPH to all microsomal P450 enzymes. Mutations in this gene cause a complex set of disorders, including apparent combined P450C17 and P450C21 deficiency, amenorrhea and disordered steroidogenesis, congenital adrenal hyperplasia and Antley-Bixler syndrome, that resemble those caused by defects in steroid metabolizing enzymes such as aromatase, 21-hydroxylase, and 17 alpha-hydroxylase. The POR c.851C>T variant is predicted to result in the amino acid substitution p.Pro284Leu. To our knowledge, this variant has not been reported any individuals with a POR-related disorder. It has been identified in unaffected individuals, and functional studies suggest it may impact the activity of POR-P450 redox partner enzymes (Agrawal et al. 2008. PubMed ID: 18551037; Huang et al. 2008. PubMed ID: 18230729; Udhane et al. 2017. PubMed ID: 28970799). This variant is reported in 0.020% of alleles in individuals of East Asian descent in gnomAD (http://gnomad.broadinstitute.org/variant/7-75612858-C-T). At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -

This sequence change replaces proline, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 284 of the POR protein (p.Pro284Leu). This variant is present in population databases (rs72557938, gnomAD 0.02%). This variant has not been reported in the literature in individuals affected with POR-related conditions. ClinVar contains an entry for this variant (Variation ID: 2632080). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt POR protein function with a negative predictive value of 80%. Experimental studies have shown that this missense change affects POR function (PMID: 18230729, 18551037, 28970799, 31669572). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
