Genes affected
RECQL4 (HGNC:9949): (RecQ like helicase 4) The protein encoded by this gene is a DNA helicase that belongs to the RecQ helicase family. DNA helicases unwind double-stranded DNA into single-stranded DNAs and may modulate chromosome segregation. This gene is predominantly expressed in thymus and testis. Mutations in this gene are associated with Rothmund-Thomson, RAPADILINO and Baller-Gerold syndromes. [provided by RefSeq, Jan 2010]

The p.Q540R variant (also known as c.1619A>G), located in coding exon 9 of the RECQL4 gene, results from an A to G substitution at nucleotide position 1619. The glutamine at codon 540 is replaced by arginine, an amino acid with highly similar properties. This amino acid position is conserved. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -

In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Not Available"; Align-GVGD: "Class C35"). This variant has not been reported in the literature in individuals with RECQL4-related conditions. This variant is present in population databases (rs754788555, ExAC 0.01%). This sequence change replaces glutamine with arginine at codon 540 of the RECQL4 protein (p.Gln540Arg). The Glutamine residue is highly conserved and there is a small physicochemical difference between glutamine and arginine . -
