The serine residue is weakly conserved and there is a moderate physicochemical difference between serine and cysteine. This variant is not present in population databases (ExAC no frequency). This missense change has been observed in individual(s) with clinical features of NBN-related conditions (PMID: 30287823). ClinVar contains an entry for this variant (Variation ID: 231991). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.S204C variant (also known as c.611C>G), located in coding exon 6 of the NBN gene, results from a C to G substitution at nucleotide position 611. The serine at codon 204 is replaced by cysteine, an amino acid with dissimilar properties. This alteration was not reported in 7051 unselected breast cancer patients and was reported with a carrier frequency of 0.00009 in 11241 female controls of Japanese ancestry (Momozawa Y et al. Nat Commun, 2018 10;9:4083). This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
