Cohen syndrome Uncertain:3
This sequence change replaces glutamic acid, which is acidic and polar, with lysine, which is basic and polar, at codon 3749 of the VPS13B protein (p.Glu3749Lys). The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This variant has not been reported in the literature in individuals affected with VPS13B-related conditions. ClinVar contains an entry for this variant (Variation ID: 657513). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Not Available"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Not Available"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
Inborn genetic diseases Uncertain:1
The p.E3749K variant (also known as c.11245G>A), located in coding exon 57 of the VPS13B gene, results from a G to A substitution at nucleotide position 11245. The glutamic acid at codon 3749 is replaced by lysine, an amino acid with similar properties. This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
VPS13B-related disorder Uncertain:1
The VPS13B c.11170G>A variant is predicted to result in the amino acid substitution p.Glu3724Lys. To our knowledge, this variant has not been reported in the literature. This variant is reported in 0.0081% of alleles in individuals of European (Non-Finnish) descent in gnomAD. At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence.
