ATM (HGNC:795): (ATM serine/threonine kinase) The protein encoded by this gene belongs to the PI3/PI4-kinase family. This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. This sequence change replaces cysteine, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 819 of the ATM protein (p.Cys819Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with ataxia-telangiectasia (PMID: 35154108). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 226114). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Variant summary: ATM c.2455T>C (p.Cys819Arg) results in a non-conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The frequency data for this variant in gnomAD is considered unreliable, as metrics indicate poor data quality at this position. c.2455T>C has been reported in the literature in at-least one individual affected with Ataxia-Telangiectasia who carries a pathogenic variant in trans (example: Blanchard-Rohner_2022). At least one publication reports experimental evidence evaluating an impact on protein function. Specifically, no detectable protein and consequently, no kinase activity was found in a lymphoblastoid cell line from an individual with Ataxia-Telangiectasia carrying the variant of interest in trans with a pathogenic null ATM variant (example: Blanchard-Rohner_2022). The following publication has been ascertained in the context of this evaluation (PMID: 35154108). ClinVar contains an entry for this variant (Variation ID: 226114). The p.C819R variant (also known as c.2455T>C), located in coding exon 15 of the ATM gene, results from a T to C substitution at nucleotide position 2455. The cysteine at codon 819 is replaced by arginine, an amino acid with highly dissimilar properties. This variant has been confirmed in trans with a ATM pathogenic variant in an individual diagnosed with ataxia telangiectasia (Blanchard-Rohner G et al. Front Immunol, 2022 Jan;13:791522). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the majority of available evidence to date, this variant is likely to be pathogenic. -
