The variant is not observed in the gnomAD v2.1.1 dataset. Predicted Consequence/Location: The variant is located in a mutational hot spot and/or well-established functional domain in which established pathogenic variants have been reported. Missense changes are a common disease-causing mechanism. In silico tool predictions suggest damaging effect of the variant on gene or gene product [REVEL: 0.64 (>=0.6, sensitivity 0.68 and specificity 0.92); 3Cnet: 0.97 (>=0.6, sensitivity 0.72 and precision 0.9)]. Same nucleotide change resulting in same amino acid change has been previously reported to be associated with GJA1 related disorder (ClinVar ID: VCV000537756 /PMID: 18946008).The variant has been observed in at least two similarly affected unrelated individuals (PMID: 18946008, 19338053, 25388818).Different missense changes at the same codon (p.Gly138Arg, p.Gly138Asp) have been reported as pathogenic/likely pathogenic with strong evidence (ClinVar ID: VCV000470216 /PMID: 12457340, 18946008). Therefore, this variant is classified as Likely pathogenic according to the recommendation of ACMG/AMP guideline. -
Oculodentodigital dysplasia, autosomal recessive Pathogenic:1
This sequence change replaces glycine, which is neutral and non-polar, with serine, which is neutral and polar, at codon 138 of the GJA1 protein (p.Gly138Ser). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with autosomal dominant oculodentodigital dysplasia (PMID: 18946008, 19338053, 25388818). ClinVar contains an entry for this variant (Variation ID: 537756). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt GJA1 protein function with a negative predictive value of 80%. This variant disrupts the p.Gly138 amino acid residue in GJA1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 18003637, 18946008, 19338053, 27226478; Invitae). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic. -
