Genes affected
KCNH2 (HGNC:6251): (potassium voltage-gated channel subfamily H member 2) This gene encodes a component of a voltage-activated potassium channel found in cardiac muscle, nerve cells, and microglia. Four copies of this protein interact with one copy of the KCNE2 protein to form a functional potassium channel. Mutations in this gene can cause long QT syndrome type 2 (LQT2). Transcript variants encoding distinct isoforms have been identified. [provided by RefSeq, May 2022]

ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 11 ACMG points.

PM1
In a hotspot region, there are 19 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 0 benign, 23 uncertain in NM_000238.4
PM2
Very rare variant in population databases, with high coverage;
PP2
Missense variant in the gene, where a lot of missense mutations are associated with disease in ClinVar. The gene has 158 curated pathogenic missense variants (we use a threshold of 10). The gene has 38 curated benign missense variants. Gene score misZ: 3.3724 (above the threshold of 3.09). Trascript score misZ: 2.4846 (below the threshold of 3.09). GenCC associations: The gene is linked to long QT syndrome 2, Brugada syndrome, short QT syndrome type 1, short QT syndrome, long QT syndrome.
PP3
MetaRNN computational evidence supports a deleterious effect, 0.989
PP5
Variant 7-150951704-C-G is Pathogenic according to our data. Variant chr7-150951704-C-G is described in ClinVar as Likely_pathogenic. ClinVar VariationId is 200368.Status of the report is criteria_provided_single_submitter, 1 stars.

The W563C likely pathogenic variant in the KCNH2 gene has been previously reported in association with LQTS; however, no specific clinical or familial segregation information was provided in either study (Nagaoka et al., 2008; Goldenberg et al., 2011). Subsequently, Miyazaki et al. (2016) identified W563C in a male and female sibling pair each diagnosed with LQTS in infancy. W563C has been identified independently in an unrelated individual with LQTS referred for genetic testing at GeneDx. The W563C variant is not observed in large population cohorts (Lek et al., 2016). W563C is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. Moreover, in silico analysis, which includes protein predictors and evolutionary conservation, supports a deleterious effect. A missense variant in the same residue (W563D) and multiple missense variants in nearby residues (A558P, A558E, L559F, L559H, A561P, A561T, A561V, H562P, H562R, L564P, A565T, C566F, C566S, I567T, W568R, W568C) have been reported in the Human Gene Mutation Database in association with LQTS (Stenson et al., 2014), supporting the functional importance of this residue and region of the protein.In summary, W563C in the KCNH2 gene is interpreted as a likely pathogenic variant. -
