NM_000249.4:c.532G>T
Variant summary
Our verdict is Pathogenic. The variant received 19 ACMG points: 19P and 0B. PVS1PM2PP3PP5_Very_Strong
The NM_000249.4(MLH1):c.532G>T(p.Glu178*) variant causes a stop gained change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. Variant has been reported in ClinVar as Pathogenic (★★). Synonymous variant affecting the same amino acid position (i.e. E178E) has been classified as Likely benign. Variant results in nonsense mediated mRNA decay.

Submissions by phenotype
Colorectal cancer, hereditary nonpolyposis, type 2 Pathogenic:1
This variant is considered pathogenic. This variant creates a termination codon and is predicted to result in premature protein truncation.
Hereditary nonpolyposis colorectal neoplasms Pathogenic:1
For these reasons, this variant has been classified as Pathogenic. Loss-of-function variants in MLH1 are known to be pathogenic. This particular variant has been reported in the literature in a family with suspected Lynch syndrome (PMID: 12624141). This sequence change creates a premature translational stop signal at codon 178 (p.Glu178*) of the MLH1 gene. It is expected to result in an absent or disrupted protein product.
