PTEN (HGNC:9588): (phosphatase and tensin homolog) This gene was identified as a tumor suppressor that is mutated in a large number of cancers at high frequency. The protein encoded by this gene is a phosphatidylinositol-3,4,5-trisphosphate 3-phosphatase. It contains a tensin like domain as well as a catalytic domain similar to that of the dual specificity protein tyrosine phosphatases. Unlike most of the protein tyrosine phosphatases, this protein preferentially dephosphorylates phosphoinositide substrates. It negatively regulates intracellular levels of phosphatidylinositol-3,4,5-trisphosphate in cells and functions as a tumor suppressor by negatively regulating AKT/PKB signaling pathway. The use of a non-canonical (CUG) upstream initiation site produces a longer isoform that initiates translation with a leucine, and is thought to be preferentially associated with the mitochondrial inner membrane. This longer isoform may help regulate energy metabolism in the mitochondria. A pseudogene of this gene is found on chromosome 9. Alternative splicing and the use of multiple translation start codons results in multiple transcript variants encoding different isoforms. [provided by RefSeq, Feb 2015]

Loss-of-function variants in PTEN are known to be pathogenic (PMID: 9467011, 21194675). This variant has been observed in an individual affected with a personal and/or family history of cancer, as well as in a family affected with Cowden syndrome (PMID: 24763289, 20223021). ClinVar contains an entry for this variant (Variation ID: 141654). This variant is present in population databases (rs587781912, ExAC 0.001%). This sequence change creates a premature translational stop signal (p.Gln17Argfs*26) in the PTEN gene. It is expected to result in an absent or disrupted protein product. For these reasons, this variant has been classified as Pathogenic. -

This deletion of 2 nucleotides in PTEN is denoted c.50_51delAA at the cDNA level and p.Gln17ArgfsX26 (Q17RfsX26) at the protein level. The normal sequence, with the bases that are deleted in brackets, is TATC[AA]GAGG. The deletion causes a frameshift, which changes a Glutamine to an Arginine at codon 17, and creates a premature stop codon at position 26 of the new reading frame. Although this variant has not, to our knowledge, been reported in the literature, it is predicted to cause loss of normal protein function through either protein truncation or nonsense-mediated mRNA decay. we consider this variant to be pathogenic. -

The c.50_51delAA pathogenic mutation, located in coding exon 1 of the PTEN gene, results from a deletion of two nucleotides at nucleotide positions 50 to 51, causing a translational frameshift with a predicted alternate stop codon (p.Q17Rfs*26). The predicted stop codon occurs within the first 150 nucleotides of thePTEN gene. This alteration may escape nonsense-mediated mRNAdecay and/or be rescued by re-initiation (Rivas et al. Science. 2015 May 8;348(6235):666-9; Lindeboom et al. Nat Genet. 2016 Oct;48(10):1112-8; Rhee et al. Sci Rep. 2017 May 10;7(1):1653). However, the impacted region is critical for protein function (Ambry internal data). As such, this alteration is interpreted as a disease-causing mutation. -
