Variant summary
Our verdict is Pathogenic. The variant received 15 ACMG points: 15P and 0B. PM1PM2PM5PP2PP5_Very_Strong
The NM_000478.6(ALPL):c.1144G>A(p.Val382Ile) variant causes a missense change. The variant allele was found at a frequency of 0.00000205 in 1,461,894 control chromosomes in the GnomAD database, with no homozygous occurrence. Variant has been reported in ClinVar as Likely pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. V382L) has been classified as Likely pathogenic.

This sequence change replaces valine, which is neutral and non-polar, with isoleucine, which is neutral and non-polar, at codon 382 of the ALPL protein (p.Val382Ile). This variant is present in population databases (rs771540767, gnomAD 0.0009%). This missense change has been observed in individual(s) with autosomal dominant hypophosphatasia and/or autosomal recessive hypophosphatasia (PMID: 9452105, 31600233). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant is also known as G1320A, Val365Ile. ClinVar contains an entry for this variant (Variation ID: 371163). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt ALPL protein function with a positive predictive value of 95%. Experimental studies have shown that this missense change affects ALPL function (PMID: 12412800). For these reasons, this variant has been classified as Pathogenic. -
