The p.V938L variant (also known as c.2812G>T), located in coding exon 17 of the CFTR gene, results from a G to T substitution at nucleotide position 2812. The valine at codon 938 is replaced by leucine, an amino acid with highly similar properties. This variant has been identified in the homozygous state and/or in conjunction with other CFTR variant(s) in individuals with features consistent with cystic fibrosis or CFTR-related disorders (Luo S et al. Gene, 2021 Jan;765:145045; Xu C et al. Pediatr Pulmonol, 2023 Oct;58:2865-2870; Wang M et al. J Assist Reprod Genet, 2024 Feb;41:505-513; Ambry internal data). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -
This sequence change replaces valine, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 938 of the CFTR protein (p.Val938Leu). This variant is present in population databases (rs749784731, gnomAD 0.03%). This missense change has been observed in individual(s) with congenital absence of vas deferens (PMID: 32777524). ClinVar contains an entry for this variant (Variation ID: 928496). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on CFTR protein function. This variant disrupts the p.Val938 amino acid residue in CFTR. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 9272157, 17329263, 28603918). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

Variant summary: CFTR c.2812G>T (p.Val938Leu) results in a conservative amino acid change located in the transmembrane domain of the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 2.4e-05 in 251402 control chromosomes, exclusively observed within the East Asian subpopulation (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.2812G>T in individuals affected with Cystic Fibrosis and no experimental evidence demonstrating its impact on protein function have been reported. No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014. Based on the evidence outlined above, the variant was classified as uncertain significance. -
