This sequence change affects a splice site in intron 40 of the TSC2 gene. It is expected to disrupt RNA splicing. Variants that disrupt the donor or acceptor splice site typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in TSC2 are known to be pathogenic (PMID: 10205261, 17304050). This variant is not present in population databases (gnomAD no frequency). Disruption of this splice site has been observed in individual(s) with tuberous sclerosis complex (PMID: 9328481). This variant is also known as 5178delTG. ClinVar contains an entry for this variant (Variation ID: 426862). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.

The c.5160+2_5160+3delTG splice site variant in the TSC2 gene has been previously reported (as c.5178delTG using alternative nomenclature) as a de novo change in an individual with TSC (Jones et al., 1997). The c.5160+2_5160+3delTG variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). This pathogenic variant destroys the canonical splice donor site of intron 40, and is expected to cause abnormal gene splicing. Therefore, this variant is interpreted to be pathogenic.
