The p.Y631C variant (also known as c.1892A>G), located in coding exon 9 of the PKP2 gene, results from an A to G substitution at nucleotide position 1892. The tyrosine at codon 631 is replaced by cysteine, an amino acid with highly dissimilar properties. This variant has been detected in cohorts with confirmed or suspected arrhythmogenic right ventricular cardiomyopathy; however, clinical details were limited and, in one family, the variant was reported to not co-segregate with phenotype (Barahona-Dussault C et al. Clin Genet, 2010 Jan;77:37-48; Rasmussen TB et al. Circ Cardiovasc Genet, 2014 Jun;7:230-40). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Arrhythmogenic right ventricular dysplasia 9 Uncertain:1
This sequence change replaces tyrosine, which is neutral and polar, with cysteine, which is neutral and slightly polar, at codon 631 of the PKP2 protein (p.Tyr631Cys). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with arrhythmogenic cardiomyopathy (PMID: 24704780). ClinVar contains an entry for this variant (Variation ID: 406548). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

In one of these families, the variant was reported not to segregate with the phenotype in the family (PMID: 24704780). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
