Variant summary
Our verdict is Likely pathogenic. The variant received 7 ACMG points: 7P and 0B. PM1PM2PP2PP3_Moderate
The NM_022041.4(GAN):c.1045C>T(p.Pro349Ser) variant causes a missense change. The variant allele was found at a frequency of 0.00000436 in 1,606,480 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. P349A) has been classified as Uncertain significance.

Inborn genetic diseases Uncertain:1
The p.P349S variant (also known as c.1045C>T), located in coding exon 6 of the GAN gene, results from a C to T substitution at nucleotide position 1045. The proline at codon 349 is replaced by serine, an amino acid with similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
Giant axonal neuropathy 1 Uncertain:1
In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt GAN protein function. ClinVar contains an entry for this variant (Variation ID: 1408531). This variant has not been reported in the literature in individuals affected with GAN-related conditions. This variant is present in population databases (rs374004725, gnomAD 0.002%). This sequence change replaces proline, which is neutral and non-polar, with serine, which is neutral and polar, at codon 349 of the GAN protein (p.Pro349Ser). -
