DICER1 (HGNC:17098): (dicer 1, ribonuclease III) This gene encodes a protein possessing an RNA helicase motif containing a DEXH box in its amino terminus and an RNA motif in the carboxy terminus. The encoded protein functions as a ribonuclease and is required by the RNA interference and small temporal RNA (stRNA) pathways to produce the active small RNA component that represses gene expression. This protein also acts as a strong antiviral agent with activity against RNA viruses, including the Zika and SARS-CoV-2 viruses. Alternative splicing results in multiple transcript variants. The p.G1364A variant (also known as c.4091G>C), located in coding exon 21 of the DICER1 gene, results from a G to C substitution at nucleotide position 4091. The glycine at codon 1364 is replaced by alanine, an amino acid with similar properties. In one study, this alteration was was observed in 2 individuals: a woman in her 30s with thyroid cancer and a man in his 40s with seminoma (Mirshahi UL et al. JAMA Netw Open, 2021 Feb;4:e210112). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.

This variant is present in population databases (no rsID available, gnomAD 0.0009%). This missense change has been observed in individual(s) with clinical features of DICER1-related conditions (PMID: 33630087). ClinVar contains an entry for this variant (Variation ID: 477180). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt DICER1 protein function with a negative predictive value of 95%.
