The p.R49H variant (also known as c.146G>A), located in coding exon 3 of the DMD gene, results from a G to A substitution at nucleotide position 146. The arginine at codon 49 is replaced by histidine, an amino acid with highly similar properties. Based on data from gnomAD, the A allele has an overall frequency of 0.003% (7/204516) total alleles studied, with 2 hemizygotes observed. The highest observed frequency was 0.016% (3/18900) of African/African-American alleles. This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
