The encoded protein is one of the components of a calcium channel, composed of a tetramer of the ryanodine receptor proteins and a tetramer of FK506 binding protein 1B proteins, that supplies calcium to cardiac muscle. The arginine at codon 1009 is replaced by tryptophan, an amino acid with dissimilar properties. This variant was detected in a reportedly healthy exome cohort (Landstrom AP et al. Circ Arrhythm Electrophysiol, 2017 Apr;10:pii: e004742) as well as in a stillbirth cohort (Sahlin E et al. PLoS ONE, 2019 Jan;14:e0210017). This alteration was also noted in a pediatric cardiomyopathy cohort (Ware SM et al. Am J Hum Genet, 2022 Feb;109:282-298). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
