Variant summary
Our verdict is Likely benign. The variant received -5 ACMG points: 0P and 5B. BP4BS2
The NM_014000.3(VCL):c.3267G>C(p.Glu1089Asp) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000192 in 1,614,040 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 15/22 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Synonymous variant affecting the same amino acid position (i.e. E1089E) has been classified as Likely benign.

Dilated cardiomyopathy 1W Uncertain:1
This sequence change replaces glutamic acid, which is acidic and polar, with aspartic acid, which is acidic and polar, at codon 1089 of the VCL protein (p.Glu1089Asp). This variant is present in population databases (rs149300310, gnomAD 0.01%). This missense change has been observed in individual(s) with clinical features of VCL-related conditions (Invitae). ClinVar contains an entry for this variant (Variation ID: 536707). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
Cardiovascular phenotype Uncertain:1
The p.E1089D variant (also known as c.3267G>C), located in coding exon 22 of the VCL gene, results from a G to C substitution at nucleotide position 3267. The glutamic acid at codon 1089 is replaced by aspartic acid, an amino acid with highly similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
