Variant summary: ATM c.4267T>C (p.Cys1423Arg) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4.8e-05 in 248814 control chromosomes. This frequency is not significantly higher than estimated for a pathogenic variant in ATM causing Ataxia-Telangiectasia (4.8e-05 vs 0.004), allowing no conclusion about variant significance. c.4267T>C has been reported in the literature in at-least one individual affected with Breast cancer without strong evidence for causality (example: Ren_2021). These report(s) do not provide unequivocal conclusions about association of the variant with Ataxia-Telangiectasia. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 30287823, 34196900). ClinVar contains an entry for this variant (Variation ID: 142407). Based on the evidence outlined above, the variant was classified as uncertain significance. -

This sequence change replaces cysteine, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 1423 of the ATM protein (p.Cys1423Arg). This variant is present in population databases (rs587782442, gnomAD 0.04%). This missense change has been observed in individual(s) with breast cancer (PMID: 34196900). ClinVar contains an entry for this variant (Variation ID: 142407). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on ATM protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.C1423R variant (also known as c.4267T>C), located in coding exon 28 of the ATM gene, results from a T to C substitution at nucleotide position 4267. The cysteine at codon 1423 is replaced by arginine, an amino acid with highly dissimilar properties. This alteration has not been reported in 7636 unselected prostate cancer patients and with a carrier frequency of 0.00008 in 12366 male controls of Japanese ancestry (Momozawa Y et al. J Natl Cancer Inst, 2020 04;112:369-376). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
