Variant summary
Our verdict is Pathogenic. The variant received 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong
The NM_000059.4(BRCA2):c.8053dupA(p.Thr2685AsnfsTer8) variant causes a frameshift change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. Variant has been reported in ClinVar as Pathogenic (★★★). Synonymous variant affecting the same amino acid position (i.e. T2685T) has been classified as Likely benign. Variant results in nonsense mediated mRNA decay.

Breast-ovarian cancer, familial, susceptibility to, 2 Pathogenic:1
Variant allele predicted to encode a truncated non-functional protein. -
Hereditary cancer-predisposing syndrome Pathogenic:1
The c.8053dupA pathogenic mutation, located in coding exon 17 of the BRCA2 gene, results from a duplication of A at nucleotide position 8053, causing a translational frameshift with a predicted alternate stop codon (p.T2685Nfs*8). This alteration was identified in a Spanish patient with bilateral breast cancer and a family history of breast cancer (Llort G et al. Hum. Mutat. 2002 Mar;19:307). Of note, this alteration is also designated as 8281insA in the published literature. In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -

Hereditary breast ovarian cancer syndrome Pathogenic:1
For these reasons, this variant has been classified as Pathogenic. Truncating variants in BRCA2 are known to be pathogenic. This particular truncation has been reported in the literature in a family with breast cancer (PMID: 11857748). This variant is also known as 8281insA in the literature. This sequence change inserts 1 nucleotide in exon 18 of the BRCA2 mRNA (c.8053dupA), causing a frameshift at codon 2685. This creates a premature translational stop signal (p.Thr2685Asnfs*8) and is expected to result in an absent or disrupted protein product. -
