ATP7B (HGNC:870): (ATPase copper transporting beta) This gene is a member of the P-type cation transport ATPase family and encodes a protein with several membrane-spanning domains, an ATPase consensus sequence, a hinge domain, a phosphorylation site, and at least 2 putative copper-binding sites. This protein is a monomer, and functions as a copper-transporting ATPase which exports copper out of the cells, such as the efflux of hepatic copper into the bile. Alternate transcriptional splice variants, encoding different isoforms with distinct cellular localizations, have been characterized. Mutations in this gene have been associated with Wilson disease which is characterized by copper accumulation. [provided by RefSeq, Dec 2019]

Variant summary: ATP7B c.111dupT (p.Ala38CysfsX3) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant allele was found at a frequency of 4e-06 in 249446 control chromosomes (gnomAD). c.111dupT has been reported in the literature in individuals affected with Wilson Disease (Coffey_2013). These data indicate that the variant is likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Two ClinVar submitters (evaluation after 2014) cite the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -

The p.Ala38CysfsX3 variant in ATP7B has been previously reported in 2 individuals with Wilson disease who were compound heterozygous for two different known pathogenic ATP7B variants (p.Asn1270Ser and p.His1069Gln) (PMID 23518715). It has been reported in ClinVar by a clinical laboratory (Variation ID 554880). This variant has also been identified in 0.0008% (1/113170) of European chromosomes by gnomAD (http://gnomad.broadinstitute.org). This variant is predicted to cause a frameshift, which alters the protein’s amino acid sequence beginning at position 38 and leads to a premature termination codon 3 amino acids downstream. This alteration is then predicted to lead to a truncated or absent protein. Loss of function of the ATP7B gene is an established disease mechanism in autosomal recessive Wilson disease. In summary, this variant meets criteria to be classified as pathogenic for autosomal recessive Wilson disease. ACMG/AMP Criteria applied: PVS1, PM2, PM3, PP4. -

A heterozygous frameshift duplication variant, NM_000053.3(ATP7B):c.111dupT, has been identified in exon 2 of 21 of the ATP7B gene. This duplication is predicted to create a frameshift starting at amino acid position 38, introducing a stop codon 3 residues downstream (NP_000044.2(ATP7B):p.Ala38Cysfs*3). This variant is predicted to result in loss of protein function either through nonsense-mediated decay, which is a reported mechanism of pathogenicity for this gene. The variant is present in the gnomAD database at a frequency of 0.00040% (1 heterozygote, 0 homozygotes). The variant has been previously described as pathogenic and reported in a patient with Wilson's disease (ClinVar, Coffey, A. J., et al. (2013)). Many up- and downstream variants also resulting in a premature termination codon have been reported in patients with Wilson's disease (ClinVar). Based on the information available at the time of curation, this variant has been classified as PATHOGENIC. -
