Variant summary: PALB2 c.2974_2975dupAT (p.Met992IlefsX2) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. Truncations downstream of this position have been classified as pathogenic by our laboratory (e.g. c.3113G>A (p.Trp1038X), c.3323delA (p.Tyr1108fsX16), c.3549C>A (p.Tyr1183X)). The variant was absent in 246244 control chromosomes (gnomAD). To our knowledge, no occurrence of c.2974_2975dupAT in individuals affected with Hereditary Breast and Ovarian Cancer and no experimental evidence demonstrating its impact on protein function have been reported. One clinical diagnostic laboratory has submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation, classifying the variant as pathogenic. Based on the evidence outlined above, the variant was classified as likely pathogenic. -
This sequence change creates a premature translational stop signal (p.Met992Ilefs*2) in the PALB2 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in PALB2 are known to be pathogenic (PMID: 17200668, 17200671, 17200672, 24136930, 25099575). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with PALB2-related conditions. ClinVar contains an entry for this variant (Variation ID: 402305). For these reasons, this variant has been classified as Pathogenic. -
Hereditary cancer-predisposing syndrome Pathogenic:1
The c.2974_2975dupAT pathogenic mutation, located in coding exon 9 of the PALB2 gene, results from a duplication of AT at nucleotide position 2974, causing a translational frameshift with a predicted alternate stop codon (p.M992Ifs*2). This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -
