Genes affected
ATP8B1 (HGNC:3706): (ATPase phospholipid transporting 8B1) This gene encodes a member of the P-type cation transport ATPase family, which belongs to the subfamily of aminophospholipid-transporting ATPases. The aminophospholipid translocases transport phosphatidylserine and phosphatidylethanolamine from one side of a bilayer to another. Mutations in this gene may result in progressive familial intrahepatic cholestasis type 1 and in benign recurrent intrahepatic cholestasis. [provided by RefSeq, Jul 2008]

Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Published functional studies demonstrate a damaging effect with this variant resulting a functionally abnormal protein (PMID: 19731236, 19918981); This variant is associated with the following publications: (PMID: 33666275, 19918981, 19381753, 14976163, 34016879, 19731236, 9500542) -

For these reasons, this variant has been classified as Pathogenic. Experimental studies have shown that this missense change affects ATP8B1 function (PMID: 14976163, 19381753, 19918981). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt ATP8B1 protein function. ClinVar contains an entry for this variant (Variation ID: 7262). This missense change has been observed in individuals with progressive familial intrahepatic cholestasis type 1 (PMID: 9500542, 33666275, 34016879). This variant is present in population databases (rs111033609, gnomAD 0.007%). This sequence change replaces glycine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 308 of the ATP8B1 protein (p.Gly308Val). -

The ATP8B1 c.923G>T variant is predicted to result in the amino acid substitution p.Gly308Val. This variant was reported in the homozygous or compound heterozygous state to be causative for intrahepatic cholestasis (Bull et al. 1998. PubMed ID: 9500542; Table S2, Hertel et al. 2021. PubMed ID: 34016879). Functional studies using cell cultures and mouse models also support the pathogenicity (Pawlikowska et al. 2004. PubMed ID: 14976163; Folmer et al. 2009. PubMed ID: 19731236; van der Velden et al. 2010. PubMed ID: 19918981). This variant is reported in 0.0065% of alleles in individuals of European (Non-Finnish) descent in gnomAD. This variant is interpreted as pathogenic. -

Variant summary: ATP8B1 c.923G>T (p.Gly308Val) results in a non-conservative amino acid change in the encoded protein sequence. Four of four in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251404 control chromosomes (gnomAD). c.923G>T has been reported in the literature in several homozygous individuals affected with Familial Intrahepatic Cholestasis (e.g. van Wessel_2021). These data indicate that the variant is very likely to be associated with disease. Publication also reported experimental evidence evaluating an impact on protein function and demonstrated highly reduced protein stability, with the absence of plasma membrane expression (Folmer_2009), in addition, mice expressing G308V/G308V showed perturbed bile salt homeostasis (Pawlikowska_2004). The following publications have been ascertained in the context of this evaluation (PMID: 19731236, 14976163, 33666275). ClinVar contains an entry for this variant (Variation ID: 7262). Based on the evidence outlined above, the variant was classified as pathogenic. -
