Variant summary

Our verdict is Uncertain significance. The variant received 0 ACMG points: 0P and 0B.

The NM_002691.4(POLD1):c.898_899delCCinsTT(p.Pro300Leu) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. The variant is present in control chromosomes in GnomAd MNV project. The variant allele was found at a frequency of 0.0000192 in 5 alleles, including 0 homozygotes. It is difficult to determine the true allele frequency of this variant because it is of type MNV, and the frequency of such variant types in population databases may be underestimated and unreliable. Variant has been reported in ClinVar as Uncertain significance (★★). Another nucleotide change resulting in the same amino acid substitution has been previously reported as Uncertain significance in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. P300S) has been classified as Uncertain significance.

Genes affected
POLD1 (HGNC:9175): (DNA polymerase delta 1, catalytic subunit) This gene encodes the 125-kDa catalytic subunit of DNA polymerase delta. DNA polymerase delta possesses both polymerase and 3' to 5' exonuclease activity and plays a critical role in DNA replication and repair. Alternatively spliced transcript variants have been observed for this gene, and a pseudogene of this gene is located on the long arm of chromosome 6. [provided by RefSeq, Mar 2012]

Variant summary: POLD1 c.898_899delinsTT (p.Pro300Leu) is a multinucleotide variant combination of 19-50905926-C-T (c.898C>T, p.Pro300Ser) and 19-50905927-C-T (c.899C>T, p.Pro300Leu) that results in a non-conservative amino acid change located in the DNA-directed DNA polymerase, family B, exonuclease domain (IPR006133) of the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 1.9e-05 in 260818 control chromosomes (gnomAD, position 19-50905926-C-T used). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.898_899delinsTT in individuals affected with Colorectal Cancer and no experimental evidence demonstrating its impact on protein function have been reported. Three submitters have cited clinical-significance assessments for this variant to ClinVar after 2014. All submitters classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -

This sequence change replaces proline, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 300 of the POLD1 protein (p.Pro300Leu). This variant is present in population databases (no rsID available, gnomAD 0.002%). This variant has not been reported in the literature in individuals affected with POLD1-related conditions. ClinVar contains an entry for this variant (Variation ID: 408010). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The POLD1 c.898_899delinsTT p.(Pro300Leu) change deletes two nucleotides and inserts two different nucleotides at position 898 to 899 to cause the substitution of the proline residue for a leucine residue at codon 300. In silico evaluation of the impact of this variant on protein function cannot be determined and functional studies have not been performed. To our knowledge, this variant has not been reported in individuals with POLD1-related disease. In summary, the evidence currently available is insufficient to determine the clinical significance of this variant. It has therefore been classified as of uncertain significance. -

The c.898_899delCCinsTT variant (also known as p.P300L), located in coding exon 7 of the POLD1 gene, results from an in-frame deletion of CC and insertion of TT at nucleotide positions 898 to 899. This results in the substitution of the proline residue for a leucine residue at codon 300, an amino acid with similar properties. Based on data from gnomAD, the TT allele has an overall frequency of 0.0019% (5/260818) total alleles studied. The highest observed frequency was 0.0042% (5/118308) of European (non-Finnish) alleles. In addition, this alteration is predicted to be deleterious by in silico analysis (Choi Y et al. PLoS ONE. 2012; 7(10):e46688). Based on the available evidence, the clinical significance of this variant remains unclear. -
