APC (HGNC:583): (APC regulator of WNT signaling pathway) This gene encodes a tumor suppressor protein that acts as an antagonist of the Wnt signaling pathway. It is also involved in other processes including cell migration and adhesion, transcriptional activation, and apoptosis. Defects in this gene cause familial adenomatous polyposis (FAP), an autosomal dominant pre-malignant disease that usually progresses to malignancy. Mutations in the APC gene have been found to occur in most colorectal cancers, where disease-associated mutations tend to be clustered in a small region designated the mutation cluster region (MCR) and result in a truncated protein product. The variant allele was found at a frequency of 4e-06 in 249802 control chromosomes, however the available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.5378C>G has been reported in the literature in one individual affected with Familial Adenomatous Polyposis (example, Kim_2019), but this report does not provide unequivocal conclusions about association of the variant with Familial Adenomatous Polyposis. Additionally, this individual was found to carry a second pathogenic variant (APC c.3577_3578del, p.Gln1193fs), providing supporting evidence for a benign role. Consistently, at-least one additional co-occurrence with another pathogenic variant has been observed at our laboratory (MSH6 c.2731C>T, p.Arg911*) further corroborating an alternative molecular basis of disease and a benign nature of this variant. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Three clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as likely benign. -
