This sequence change affects an acceptor splice site in intron 10 of the DSP gene. It is expected to disrupt RNA splicing. Variants that disrupt the donor or acceptor splice site typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in DSP are known to be pathogenic (PMID: 20716751, 24503780, 25227139). This variant is not present in population databases (gnomAD no frequency). Disruption of this splice site has been observed in individual(s) with clinical features of DSP-related conditions (PMID: 30453078, 31317183, 36043215). ClinVar contains an entry for this variant (Variation ID: 534305). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -

The c.1267-2A>G intronic variant results from an A to G substitution two nucleotides upstream from coding exon 11 in the DSP gene. This variant has been detected in an individual from an arrhythmogenic right ventricular cardiomyopathy cohort, and an individual from a dilated cardiomyopathy cohort; however, details were limited (Poloni G et al. Heart Rhythm, 2019 05;16:773-780; Augusto JB et al. Eur Heart J Cardiovasc Imaging, 2020 Mar 1;21(3):326-336). Alterations that disrupt the canonical splice site are expected to result in aberrant splicing. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site and will result in the creation or strengthening of a novel splice acceptor site. The resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNAdecay; however, direct evidence is unavailable. The exact functional effect of the missing amino acids is unknown. This nucleotide position is highly conserved in available vertebrate species. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
