CDKN2A (HGNC:1787): (cyclin dependent kinase inhibitor 2A) This gene generates several transcript variants which differ in their first exons. At least three alternatively spliced variants encoding distinct proteins have been reported, two of which encode structurally related isoforms known to function as inhibitors of CDK4 kinase. The remaining transcript includes an alternate first exon located 20 Kb upstream of the remainder of the gene; this transcript contains an alternate open reading frame (ARF) that specifies a protein which is structurally unrelated to the products of the other variants. This ARF product functions as a stabilizer of the tumor suppressor protein p53 as it can interact with, and sequester, the E3 ubiquitin-protein ligase MDM2, a protein responsible for the degradation of p53. In spite of the structural and functional differences, the CDK inhibitor isoforms and the ARF product encoded by this gene, through the regulatory roles of CDK4 and p53 in cell cycle G1 progression, share a common functionality in cell cycle G1 control. This gene is frequently mutated or deleted in a wide variety of tumors, and is known to be an important tumor suppressor gene. [provided by RefSeq, Sep 2012]

The p.H83Q pathogenic mutation (also known as c.249C>G), located in coding exon 2 of the CDKN2A gene, results from a C to G substitution at nucleotide position 249. The histidine at codon 83 is replaced by glutamine, an amino acid with highly similar properties. Another alteration at the same codon, p.H83Q (c.249C>A) resulting in the same amino acid change, has been reported in multiple individuals with melanoma as well as an Italian familial melanoma kindred (Begg CB et al. J Natl Cancer Inst. 2005 Oct 19;97(20):1507-15; Berwick M et al. Cancer Epidemiol Biomarkers Prev. 2006 Aug;15(8):1520-5; Orlow I et al. J Invest Dermatol. 2007 May;127(5):1234-43; Pedace L et al. Cancer Epidemiol. 2011 Dec;35(6):e116-20; Ambry internal data). This amino acid position is highly conserved in available vertebrate species. This alteration disrupts the ankyrin domain near its interface with kinases (Russo AA. et al. Nature.1998 Sep;395(6699):237-43; Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the supporting evidence, this alteration is interpreted as a disease-causing mutation. -

The CDKN2A gene encodes two different proteins, p16INK4a and p14ARF, which are translated from alternative transcripts with different open reading frames. Both transcripts have been analyzed. We report either the variant with the higher classification or default to the CDKN2A (p16INK4a) variant. This report therefore includes the details for the CDKN2A (p16INK4a) variant. This sequence change replaces histidine, which is basic and polar, with glutamine, which is neutral and polar, at codon 83 of the CDKN2A (p16INK4a) protein (p.His83Gln). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with sporadic or familial melanoma (PMID: 16234564, 16896043, 17218939, 21462282, 21893440, 28146043). This variant is also known as c.292C>G (p.Arg98Gly) in the CDKN2A (p14ARF) transcript. ClinVar contains an entry for this variant (Variation ID: 376381). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
