Genes affected
KMT2D (HGNC:7133): (lysine methyltransferase 2D) The protein encoded by this gene is a histone methyltransferase that methylates the Lys-4 position of histone H3. The encoded protein is part of a large protein complex called ASCOM, which has been shown to be a transcriptional regulator of the beta-globin and estrogen receptor genes. Mutations in this gene have been shown to be a cause of Kabuki syndrome. [provided by RefSeq, Oct 2010]

Kabuki syndrome 1 Pathogenic:2
Pathogenic, criteria provided, single submitterclinical testingNeuberg Centre For Genomic Medicine, NCGM-The splice donor c.14075+1G>A variant in the KMT2D gene has been observed in individual(s) with Kabuki syndrome (Dillon, Oliver James et al., 2018). The variant is absent in gnomAD Exomes. The variant has been submitted to ClinVar databse as Likely Pathogenic/Pathogenic. The variant affects the GT donor splice site downstream of exon 44. Loss of function variants have been previously reported to be disease causing. The spliceAI tool predicts the variant to be damaging. For these reasons, this variant has been classified as Pathogenic. -
Pathogenic, criteria provided, single submitterclinical testingVictorian Clinical Genetics Services, Murdoch Childrens Research InstituteNov 24, 2014This substitution is predicted to abolish the donor splice site of exon 43 and may result in premature truncation of the protein. This is a novel variant and is predicted to be deleterious by in-silico models. It was confirmed to be de novo in a child with clinical features of Kabuki syndrome. -
Kabuki syndrome Pathogenic:1
Likely pathogenic, criteria provided, single submitterclinical testingInvitaeMar 02, 2022In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. ClinVar contains an entry for this variant (Variation ID: 369674). Disruption of this splice site has been observed in individual(s) with Kabuki syndrome (PMID: 29453417; Invitae). This variant is not present in population databases (gnomAD no frequency). This sequence change affects a donor splice site in intron 43 of the KMT2D gene. It is expected to disrupt RNA splicing. Variants that disrupt the donor or acceptor splice site typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in KMT2D are known to be pathogenic (PMID: 22126750). -
Inborn genetic diseases Pathogenic:1
Pathogenic, criteria provided, single submitterclinical testingAmbry GeneticsMay 02, 2022The c.14075+1G>A intronic variant results from a G to A substitution one nucleotide after coding exon 43 of the KMT2D gene. Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. This variant was not reported in population-based cohorts in the Genome Aggregation Database (gnomAD). This alteration was reported in an infant with sensorineural hearing impairment, cleft palate, ventricular septal defect, and partially duplicated displaced kidney (Stark, 2016). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. Based on the available evidence, this alteration is classified as pathogenic. -
