Variant summary
Our verdict is Likely benign. The variant received -6 ACMG points: 0P and 6B. BP4BP6BS2
The NM_000368.5(TSC1):c.3184C>T(p.Arg1062Trp) variant causes a missense change. The variant allele was found at a frequency of 0.000136 in 1,614,048 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R1062Q) has been classified as Likely benign.

Tuberous sclerosis 1 Uncertain:1Benign:5
This variant is considered likely benign. This variant has been observed at a population frequency that is significantly greater than expected given the associated disease prevalence and penetrance. -

This variant was observed as part of a predisposition screen in an ostensibly healthy population. A literature search was performed for the gene, cDNA change, and amino acid change (where applicable). Publications were found based on this search. The evidence from the literature, in combination with allele frequency data from public databases where available, was sufficient to determine this variant is unlikely to cause disease. Therefore, this variant is classified as likely benign. -
not specified Uncertain:2
The TSC1 c.3184C>T; p.Arg1062Trp variant (rs118203745) is reported in the literature in an individual with tuberous sclerosis (Qin 2010). However, functional assays demonstrate the variant protein behaves similarly to wild type protein (Hoogeveen-Westerveld 2011). This variant is reported with conflicting interpretations in ClinVar (Variation ID: 49015). It is found in the general population with an overall allele frequency of 0.02% (44/282762 alleles) in the Genome Aggregation Database. The arginine at codon 1062 is moderately conserved, and computational analyses (SIFT, PolyPhen-2) predict that this variant may be deleterious. However, due to limited information, the clinical significance of this variant is uncertain at this time. References: Hoogeveen-Westerveld M et al. Variant identified in a genome or exome case(s) and assessed due to predicted null impact of the variant or pathogenic assertions in the literature or databases. Disclaimer: This variant has not undergone full assessment. The following are preliminary notes: ClinVar: 1 LB; Reported in one proband, functional studies do not support pathogenicity -
Hereditary cancer-predisposing syndrome Benign:2
- -
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. -

Isolated focal cortical dysplasia type II Benign:1
This variant was observed as part of a predisposition screen in an ostensibly healthy population. A literature search was performed for the gene, cDNA change, and amino acid change (where applicable). No publications were found based on this search. Allele frequency data from public databases was too high to be consistent with this variant causing disease. Therefore, this variant is classified as benign. -
TSC1-related disorder Benign:1
This variant is classified as likely benign based on ACMG/AMP sequence variant interpretation guidelines (Richards et al. 2015 PMID: 25741868, with internal and published modifications). -
