Variant summary

Our verdict is Likely pathogenic. The variant received 9 ACMG points: 10P and 1B. PS3PM1PM5PP2PP5BP4

The NM_000492.4(CFTR):c.1054C>T(p.Arg352Trp) variant causes a missense change. The variant allele was found at a frequency of 0.0000849 in 1,613,830 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). ClinVar reports functional evidence for this variant: "SCV000052119: In addition, other functional studies investigating the Arg352 residue have reported that this region is important for stabilizing the channel's tertiary structure and predict that mutations in this amino acid may have a functional impact on the protein (e.g. Guinamard_1999, Cui_2008). Finally, when expressed alone in vitro, channel Cl- conductance and overall protein expression were reduced to <10% of wild type controls (e.g. Bihler_2024)." and additional evidence is available in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R352Q) has been classified as Pathogenic.

Genes affected
CFTR (HGNC:1884): (CF transmembrane conductance regulator) This gene encodes a member of the ATP-binding cassette (ABC) transporter superfamily. The encoded protein functions as a chloride channel, making it unique among members of this protein family, and controls ion and water secretion and absorption in epithelial tissues. Channel activation is mediated by cycles of regulatory domain phosphorylation, ATP-binding by the nucleotide-binding domains, and ATP hydrolysis. Mutations in this gene cause cystic fibrosis, the most common lethal genetic disorder in populations of Northern European descent. The most frequently occurring mutation in cystic fibrosis, DeltaF508, results in impaired folding and trafficking of the encoded protein. Multiple pseudogenes have been identified in the human genome. [provided by RefSeq, Aug 2017]

ACMG classification

Classification was made for transcript

Our verdict: Likely_pathogenic. The variant received 9 ACMG points.

PS3
PS3 evidence extracted from ClinVar submissions: SCV000052119: In addition, other functional studies investigating the Arg352 residue have reported that this region is important for stabilizing the channel's tertiary structure and predict that mutations in this amino acid may have a functional impact on the protein (e.g. Guinamard_1999, Cui_2008). Finally, when expressed alone in vitro, channel Cl- conductance and overall protein expression were reduced to <10% of wild type controls (e.g. Bihler_2024).; SCV002715936: "In multiple assays testing CFTR function, this variant showed functionally abnormal results (CFTR2; Bihler H et al. J Cyst Fibros, 2024 Jul;23:664-675)."; SCV000603070: chloride channel activity of 11% of wildtype (CFTR2 database).
PM1
In a hotspot region, there are 8 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 1 benign, 24 uncertain in NM_000492.4
PM5
Other missense variant is known to change same aminoacid residue: Variant chr7-117540285-G-A is described in ClinVar as Pathogenic|drug_response. ClinVar VariationId is 7198.Status of the report is reviewed_by_expert_panel, 3 stars.
PP2
Missense variant in the gene, where a lot of missense mutations are associated with disease in ClinVar. The gene has 197 curated pathogenic missense variants (we use a threshold of 10). The gene has 46 curated benign missense variants. Gene score misZ: -3.1397 (below the threshold of 3.09). Trascript score misZ: -1.0868 (below the threshold of 3.09). GenCC associations: The gene is linked to hereditary chronic pancreatitis, cystic fibrosis, congenital bilateral absence of vas deferens.
PP5
Variant 7-117540284-C-T is Pathogenic according to our data. Variant chr7-117540284-C-T is described in ClinVar as Conflicting_classifications_of_pathogenicity. ClinVar VariationId is 35816.
BP4
Computational evidence support a benign effect (MetaRNN=0.04657367). . Strength limited to SUPPORTING due to the PP5.
