Variant summary
Our verdict is Uncertain significance. The variant received 3 ACMG points: 3P and 0B. PM1PP2
The NM_000497.4(CYP11B1):c.1015_1016delGCinsAT(p.Ala339Ile) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. The variant is present in control chromosomes in GnomAd MNV project. The variant allele was found at a frequency of 0.000039 in 11 alleles, including 0 homozygotes. It is difficult to determine the true allele frequency of this variant because it is of type MNV, and the frequency of such variant types in population databases may be underestimated and unreliable. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. A339V) has been classified as Uncertain significance.

Variant summary: CYP11B1 c.1015_1016delinsAT (p.Ala339Ile) results in a non-conservative amino acid change in the encoded protein sequence. Three of four in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4.3e-05 in 282350 control chromosomes. This frequency is not significantly higher than estimated for a pathogenic variant in CYP11B1 causing Congenital Adrenal Hyperplasia (4.3e-05 vs 0.002), allowing no conclusion about variant significance. To our knowledge, no occurrence of c.1015_1016delinsAT in individuals affected with Congenital Adrenal Hyperplasia has been reported. At least one publication reports experimental evidence evaluating an impact on protein function. These results showed no damaging effect of this variant (bottnerb_1998). The following publication has been ascertained in the context of this evaluation (PMID: 9546661). ClinVar contains an entry for this variant (Variation ID: 35979). Based on the evidence outlined above, the variant was classified as uncertain significance. -

This sequence change replaces alanine, which is neutral and non-polar, with isoleucine, which is neutral and non-polar, at codon 339 of the CYP11B1 protein (p.Ala339Ile). The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This variant has not been reported in the literature in individuals affected with CYP11B1-related conditions. ClinVar contains an entry for this variant (Variation ID: 35979). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
