APC (HGNC:583): (APC regulator of WNT signaling pathway) This gene encodes a tumor suppressor protein that acts as an antagonist of the Wnt signaling pathway. It is also involved in other processes including cell migration and adhesion, transcriptional activation, and apoptosis. Defects in this gene cause familial adenomatous polyposis (FAP), an autosomal dominant pre-malignant disease that usually progresses to malignancy. Mutations in the APC gene have been found to occur in most colorectal cancers, where disease-associated mutations tend to be clustered in a small region designated the mutation cluster region (MCR) and result in a truncated protein product. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 2e-05 in 394116 control chromosomes, predominantly at a frequency of 4.5e-05 (8/177,752 alleles) within the Non-Finnish European subpopulation in the gnomAD database (gnomAD v2.1 exomes dataset and gnomAD v3 genomes dataset). This frequency is somewhat lower than the estimated maximal expected allele frequency of a pathogenic APC variant (7.1e-05). The variant was reported in the literature as an incidental finding in 1/572 individuals in an exome sequencing study (i.e. the ClinSeq study, involving participants of 45-65 years of age, selected for a range of atherosclerosis phenotypes, but not for personal or family histories of cancer) (Johnston_2012). To our knowledge, no occurrence of c.2677G>A in individuals affected with Familial Adenomatous Polyposis and no experimental evidence demonstrating its impact on protein function have been reported. Four other clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation, and all laboratories classified the variant as uncertain significance.
