It is also involved in other processes including cell migration and adhesion, transcriptional activation, and apoptosis. Defects in this gene cause familial adenomatous polyposis (FAP), an autosomal dominant pre-malignant disease that usually progresses to malignancy. Mutations in the APC gene have been found to occur in most colorectal cancers, where disease-associated mutations tend to be clustered in a small region designated the mutation cluster region (MCR) and result in a truncated protein product. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 2e-05 in 394116 control chromosomes, predominantly at a frequency of 4.5e-05 (8/177,752 alleles) within the Non-Finnish European subpopulation in the gnomAD database (gnomAD v2.1 exomes dataset and gnomAD v3 genomes dataset). This frequency is somewhat lower than the estimated maximal expected allele frequency of a pathogenic APC variant (7.1e-05). The variant was reported in the literature as an incidental finding in 1/572 individuals in an exome sequencing study (i.e. the ClinSeq study, involving participants of 45-65 years of age, selected for a range of atherosclerosis phenotypes, but not for personal or family histories of cancer) (Johnston_2012). To our knowledge, no occurrence of c.2677G>A in individuals affected with Familial Adenomatous Polyposis and no experimental evidence demonstrating its impact on protein function have been reported. Four other clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation, and all laboratories classified the variant as uncertain significance.
