This missense variant replaces arginine with histidine at codon 382 of the MSH2 protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in individuals affected with microsatellite high colorectal cancer (PMID: 18257912). This variant has been identified in 2/251460 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.R382H variant (also known as c.1145G>A), located in coding exon 7 of the MSH2 gene, results from a G to A substitution at nucleotide position 1145. The arginine at codon 382 is replaced by histidine, an amino acid with highly similar properties. This variant has been reported in the literature in a Chinese proband with a MSI-high colorectal cancer at age 61 (Jin HY et al. BMC Cancer, 2008;8:44). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

This variant is denoted MSH2 c.1145G>A at the cDNA level, p.Arg382His (R382H) at the protein level, and results in the change of an Arginine to a Histidine (CGT>CAT). This variant has been reported in at least one individual with late-onset microsatellite unstable (MSI-H) rectal cancer (Jin 2008). The International Society for Gastrointestinal Hereditary Tumours Incorporated (InSiGHT) classifies this variant as a variant of uncertain significance (Thompson 2014). MSH2 Arg382His was not observed at a significant allele frequency in large population cohorts (Lek 2016). This variant is located in the Lever domain and within a region responsible for interaction with MSH6 and MSH3 (Guerrette 1998, L?tzen 2008, Kansikas 2011). In silico analysis, which includes protein predictors and evolutionary conservation, supports a deleterious effect. Based on currently available evidence, it is unclear whether MSH2 Arg382His is a pathogenic or benign variant. We consider it to be a variant of uncertain significance. -
