This missense variant replaces glutamine with histidine at codon 1107 of the BRCA2 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been detected in at least one individual fulfilling the National Comprehensive Cancer Network's hereditary breast and ovarian cancer testing criteria (PMID: 29161300). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.Q1107H variant (also known as c.3321A>C), located in coding exon 10 of the BRCA2 gene, results from an A to C substitution at nucleotide position 3321. The glutamine at codon 1107 is replaced by histidine, an amino acid with highly similar properties. This alteration has been identified in one individual meeting NCCN criteria for hereditary breast/ovarian cancer genetic testing, however, specific clinical details were not provided (Alemar B et al. PLoS ONE, 2017 Nov;12:e0187630). Additionally, this alteration has been reported in multiple individuals diagnosed with breast and/or ovarian cancer (Herzog JS et al. NPJ Breast Cancer, 2021 Aug;7:107). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this alteration remains unclear. -

Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 238670 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.3321A>C has been reported in the literature in individuals with personal and/or family history of breast and/or ovarian cancer, without strong evidence for or against causality (Alemar_2017, Herzog_2021). These reports therefore do not provide unequivocal conclusions about association of the variant with Hereditary Breast and Ovarian Cancer Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Three other clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014, and all of them classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -
