The ATM c.6420C>A (p.Phe2140Leu) variant has been reported in the published literature in an individual with pancreatic cancer (PMID: 35171259 (2022)). This variant has also been identified as a somatic variant in an individual with endometrial cancer (PMID: 34954471 (2022)). The frequency of this variant in the general population, 0.000087 (3/34522 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is damaging. Based on the available information, we are unable to determine the clinical significance of this variant. -

The p.F2140L variant (also known as c.6420C>A), located in coding exon 43 of the ATM gene, results from a C to A substitution at nucleotide position 6420. The phenylalanine at codon 2140 is replaced by leucine, an amino acid with highly similar properties. This variant was identified in 1 of 1009 patients amongst a cohort of Chinese patients with a personal history of pancreatic ductal adenocarcinoma (Yin L et al. JAMA Netw Open, 2022 Feb;5:e2148721). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -
